What is a Elder Care Coordinator?

What is a Elder Care Coordinator?  How can a ECC help with our concerns about our loved one? Will the Elder Care Coordinator help us understand how the long-term care system works and how long-term and health care services for our loved one are accessed and funded? Will the ECC know about community resources? Can the ECC help the people who work with our loved one work together as a team?

Your Elder Care Coordinator

The ECC specializes in assisting people and their families to attain the highest quality of life given their circumstances. A ECC will:

  • Conduct care assessments to identify problems and assist in solving them.
  • Assist families in identifying and arranging in-home help or other services.
  • Coordinate with medical and health providers.
  • Review medical issues and other referrals to other specialists to provide appropriate care while conserving financial resources.
  • Provide support, guidance, and advocacy during a crisis.
  • Help with coordinating transfer and transportation of an older person to or from a retirement complex, assisted care living facility, or skilled nursing facility.
  • Call for, and attend care meeting in facilities to meet the needs of the elderly client.
  • Provide education, including guiding the client and family through the maze of long-term care issues.
  • Offer counseling and support.
  • Advocate for the client
  • Prepare and guide clients and families through the Medicaid application process.

What Does this Mean to You?

As a part of the Life Care Plan, a Elder Care Coordinator is assigned to help our clients and their families with their long-term care concerns. At Dafoe Law, PLLC, your ECC functions as the point of contact for the family and assist in coordinating services to help you take care of  your loved one.

Your ECC has extensive knowledge about the costs, quality, and availability of resources in the community. As our families begin their journey through the long-term care system, it is helpful for them to have supportive and knowledgable advocate to accompany them along the way.
